Skilled Nursing Services

Professional nursing services are provided by both registered nurses (RNs) and licensed practical nurses (LPNs). They perform a variety of tasks, including wound care, dressing change, treatments and injections, IV therapy, diabetic care, catheter care, cardiac and respiratory assessment and care, medication management, pain management, and patient/ family health teaching.

Physical Therapy Services

Physical therapists focus on strengthening the lower body in order to improve endurance and mobility and are oftentimes critical to post surgery recovery. PT develops rehabilitative plans of care, collaborate with the attending MD to obtain orders, provide education to patients and families and set achievable goals to discharge.

Occupational Therapy

Occupational therapists focus primarily on strengthening the upper body and helping patients learn to safely complete tasks of daily living, such as showering, eating and dressing. OT develops rehabilitative plans of care, collaborate with the attending MD to obtain orders, provide education to patients and families and set achievable goals to discharge.

Speech/Language Therapy

Speech therapists help patients regain abilities following a stroke and also help patients who have difficulties swallowing as a result of a medical event. ST develops rehabilitative plans of care, collaborate with the attending MD to obtain orders, provide education to patients and families and set achievable goals to discharge.

Medical Social Services

Medical Social Services are provided in accordance with the plan of care and are services by qualified licensed social workers. LISW (and SWA under the LISW) manage the psychosocial needs of patients, assist with financial planning, collaborate with the attending MD to obtain orders, assist with community resources needed by patients, and provide counseling according to the plan of care.

Home Health Aide Services

Home Health Aides assist with personal care and hygiene, assist with ambulation, transfers and exercise programs, provide basic nutritional support, provide light housekeeping tasks related to the patient. HHAs work under the supervision of a registered nurse and follow a plan of care that has been established by the nurse.

In addition to certified Medicare services, our Agency offer non-certified homecare which includes, private pay, Homemakers, companions, Personal Care Attendants, HHA’s, Nursing, PT, SLP and OT services. Under the non-certified business lines, our Agency will provide service contracts under certain contractual work with state subsidized elder service agencies.

Companion

Companion services include support, encouragement, companionship, respite breaks for family or caregivers, and provision of and instruction in reporting of signs, symptoms, and/or changes in the patient's condition. The Companion is assigned to a patient by the coordinator and is given a written assignment sheet. The Companion reports directly to a qualified supervisor.

PCA/Live-In

The PCA/ Live-in services includes, assistance with nutritional and environmental support, personal hygiene, feeding and dressing. The PCA reports to the Scheduling Coordinator, Nursing/HHA Supervisor. The PCA/ Live-ins are qualified and have appropriate clinical supervision. They also must have a criminal background check and a current CPR certification.

Home Making

Homemaking services include dietary management, household services essential to the patient's health and wellbeing, and provision of and instruction in reporting of signs, symptoms, and/or changes in the patient's condition. The homemaker is assigned to a patient by the coordinator and is given a written assignment sheet. The Homemaker reports directly to the Scheduling Coordinator.
